Self-employment on its own does not establish participation in public health insurance - unlike employment with a Czech company.
Health insurance is submitted with an application for a long-term visa, for residence and for its extension.
You receive the policy and proof of payment by email, usually in English as well.
If you are also employed by a Czech company alongside your business, you are in the public system and do not need commercial insurance. Check this before you arrange anything.
A partner and children are assessed on their own. Children under 18 with long-term residence have been in the public system since 1 January 2024.
